Custodial Request

If you have a custodial request please fill out this form and we will attend to it as quickly as possible. (Please provide a suffiencent amount of time to schedule work in.) To better serve you please provide your name, extension, building name, and room number. Also, note that all work orders will be confirmed by telephone. Any work order sent without a telephone contact number will not be processed.

You may also contact the Facilities Services Customer Services for routine work request and work tickets. For requests that need immediate attention please contact our customer service center at 346-2319.
