All Modified Work Offer forms are to be returned directly to the Facilities Services Safety Manager rather than Environmental Health & Safety.
This website includes information for injured employees and their supervisors.  The SAIF Workers Compensation Claim Form (801), the Employee Status Report (ESR), and the Modified Work Job Offer can be obtained from this site.  Please use the Supervisor's Accident Investigation Report (SAIR) that is available on the Facilities Services site rather than the one available through the Environmental Health & Safety site.  All Workers Compensation forms are to be returned directly to the Facilities Services Safety Manager rather than Environmental Health & Safety.
